Blaire House of Worcester is seeking an energetic Licensed Social Worker to uphold our high standards for elder care. Our 75-bed skilled nursing center specializes in dementia care, and we are family owned and operated. You will develop social histories, provide counseling as a patient advocate, maintain progress notes in medical records, and craft discharge plans with community services; a licensed LSW or higher and at least one year of long-term care experience are required. #J-18808-Ljbffr Blaire House of Worcester
