We also place a strong emphasis on employee well-being, professional fulfillment, engagement, and work-life balance.The Family Medicine Nurse Practitioner (NP) is an integral member of the Vanguard Medical Group care team, providing comprehensive primary care services to patients across the lifespan. This role includes preventive care, assessment and diagnosis, management of acute and chronic conditions, prescribing and managing treatment, and patient and family education. The Nurse Practitioner works collaboratively with physicians, nurses, and other members of the clinical team to deliver coordinated, patient-centered care that supports Vanguard Medical Group's commitment to improving the health of the communities we serve.Hours: Full-timeResponsibilitiesClinical Care and Patient ManagementConduct comprehensive patient assessments, including obtaining medical and family histories, reviewing patient records, evaluating presenting concerns and risk factors, and performing appropriate physical examinations.Diagnose, treat, and manage acute and chronic medical conditions for pediatric, adult, and geriatric patients within the Nurse Practitioner's scope of practice.Order, perform, and interpret appropriate laboratory studies, diagnostic testing, imaging, screenings, and other clinical evaluations.Consult and collaborate with physicians and other clinicians when additional evaluation or clinical guidance is appropriate.Develop and implement individualized treatment plans based on clinical findings, diagnoses, patient needs, and evidence-based standards of care.Prescribe and manage medications and treatments in accordance with practice guidelines, organizational policies, and applicable New Jersey requirements.Administer and document immunizations, injections, wound care, and other minor clinical procedures within scope of practice.Provide patient and family education regarding medications, treatment plans, disease prevention, wellness, health promotion, and ongoing care needs.Identify appropriate resources and referrals to support patient and family needs.Provide monitoring, follow-up, and continuity of care between visits, including responding to and triaging patient calls and electronic communications.Accurately and promptly document assessments, examination findings, test results, diagnoses, medications, treatment plans, and follow-up care in the EMR.Quality and Performance Accountability (QIP)Partner with the clinical team to meet Quality Incentive Program (QIP) benchmarks, including preventive screenings, immunization rates, and chronic disease management metrics.Review patient panels and proactively identify and close care gaps using EMR tools, reports, and population health resources.Participate in QIP, population health, peer review, chart review, and other quality assurance and clinical performance improvement activities.Contribute to accurate clinical and risk-adjustment documentation to support quality outcomes and continuity of care.Provide appropriate outreach and follow-up for high-risk patients and patients requiring additional support to improve health outcomes.Support the development and implementation of clinical guidelines, policies, and evidence-based standards of care.Collaboration and Team EngagementCollaborate closely with physicians and other members of the clinical team in managing patients with acute, chronic, and long-term healthcare needs.Participate in team huddles, meetings, case reviews, and other team-based care initiatives to improve coordination of care.Serve as a clinical resource and collaborator for members of the care team.Communicate relevant patient care and case management information to appropriate healthcare professionals.Provide input into the development and improvement of practice workflows and clinical protocols.Support the training and onboarding of clinical team members, as appropriate.Compliance and Professional StandardsMaintain patient confidentiality and comply with HIPAA and all applicable privacy requirements.Adhere to infection-control, OSHA, safety, clinical, and organizational policies and procedures.Practice in accordance with applicable New Jersey Advanced Practice Nurse requirements, including any collaborative or joint protocol requirements applicable to the individual practitioner.Maintain required professional licensure, certifications, registrations, and continuing education.Stay current with evidence-based practices and developments in family medicine and primary care.Promote patient advocacy, quality outcomes, and continuity of care throughout the patient experience.QualificationsQualifications:Master's degree in nursing from an accredited Nurse Practitioner program.Current national certification as a Family Nurse Practitioner (FNP) through a recognized certifying organization.Current New Jersey Registered Nurse license or qualifying multistate RN privilege, as applicable.Current New Jersey Advanced Practice Nurse (APN) certification with prescriptive authority.Current New Jersey Controlled Dangerous Substances (CDS) registration and federal DEA registration, or ability to obtain as required for the position.BLS certification required; ACLS certification required or obtained within 3 months of hire1 year of Nurse Practitioner experience preferred, with family medicine or primary care experience strongly preferredDemonstrated ability to manage a diverse panel of patients and support quality improvement initiatives.Strong EMR skills (Athena experience preferred) and ability to use data to drive outcomes.Excellent interpersonal, communication, and organizational skills.Core Competencies:Patient-Centered Care & Health PromotionClinical Excellence & ThoroughnessData-Driven Quality ImprovementMedical Teamwork & CollaborationPatient Education & Motivational CounselingInfection Control & Safety PracticesAdministering Medications & Minor ProceduresContinuous Self-Development & Professional GrowthPhysical Requirements:Must be able to sit, stand, and walk for extended periods.May require light lifting (up to 20 lbs.) for supplies or office materials.Must have visual and auditory acuity to operate computer systems and phones.Compensation and Benefits:The anticipated annual compensation for this position ranges from $115,000-$140,000, depending on the role and experience level.
