Registered Nurse (RN) Case Manager – Home Health | South Dallas & Surrounding Areas

Ultimate Home Health Care is seeking dedicated RN Case Managers to provide high-quality home healthcare to clients across the Dallas-Fort Worth metroplex and surrounding areas. Role Responsibilities:

Provide professional, compassionate care in clients’ homes
Manage diverse client needs with Home Health expertise
Document care accurately using OASIS/Kinnser
Respond to PRN emergency calls as needed

Qualifications:

Graduate of an accredited Nursing Program
Active Texas RN License and Driver’s License
Minimum 1 year of Home Health experience
Experience with OASIS/Kinnser required
Ability to work flexible schedules and travel locally

Work Location: Client homes throughout Dallas and surrounding areas
