Job Location: Hobart Nursing Center - Hobart, OK 73651, Position Type: Full Time, Job Category: Health Care, The Licensed Practical Nurse (LPN) provides direct nursing care to residents in a skilled nursing facility under the supervision of a Registered Nurse (RN) or Director of Nursing. The LPN is responsible for administering medications, monitoring resident health, assisting with care plans, and ensuring high-quality care in accordance with facility policies and state regulations.

Key Responsibilities

• Provide direct nursing care to residents in accordance with care plans and physician orders.
• Administer medications and treatments as prescribed.
• Monitor residents’ vital signs, conditions, and responses to treatment.
• Report changes in resident condition to the Registered Nurse or physician.
• Assist with the development and implementation of individualized care plans.
• Supervise and support Certified Nursing Assistants (CNAs).
• Maintain accurate and timely medical documentation.
• Ensure compliance with infection control and safety standards.
• Respond to resident needs and emergencies.
• Participate in shift reports and interdisciplinary team meetings.
• Provide compassionate care while maintaining resident dignity and privacy.

Qualifications: Qualifications

• Current Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) license in the state of employment.
• Graduate of an accredited practical nursing program.
• Experience in long-term care or skilled nursing preferred.
• Current CPR certification required.
