TravelNurseSource is working with Cynet Health to find a qualified Home Health RN in Phoenix, Arizona, 85012!

Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum of 1 year in Adult Home Health or Hospice experience License: Active Nursing License required Certifications: CPR, BLS Must-Have: Must have reliable transportation and a current driver's license. Must have agency-required liability insurance. Must be proficient in HomeCare HomeBase and OASIS. Must have recent specialty experience. Description: Provides and coordinates home nursing care according to physician's orders. Responsible for the competent application of the nursing process. Direct supervision and evaluation of Home Health Aides and Licensed Practical Nurses. Home visits and periodic office meetings are required. Must be willing to travel up to 75 miles to visit patients. Mileage will be reimbursed at the IRS standard rate. Contractor will be assigned between 5-10 patients per day. Each type of visit and activity holds a different point value. The contractor should expect to achieve 30 productivity points per week. Types of cases include Start of Care, Wound Care, and Follow-up Visits. Documentation must be completed within 24 hours of patient appointments. All clinicians must accept patients scheduled, regardless of distance. On-call responsibilities may vary. Daily travel may cover approximately 400 miles per week. No guaranteed hours are provided in this contract. All travelers must have proficient electronic medical record experience. Acceptable candidates must not have been direct employees of any affiliated locations. Local applicants will be accepted at the same rate.
