Job Summary and Qualifications Full time (Monday - Friday, days), no nights, no on-call, no weekends, no holidays Candidate Profile Seeking a Licensed Social Worker who performs evaluations and services to patients of various age groups. Collects data and renders a professional opinion about treatment and care. What you will do Perform vocational rehabilitation assessment in a timely manner; dictate interdisciplinary report to clinical record within the required time frames. Initiate and discharge documents within the required timeframe. Document therapy notes on the clinical record. Formulate and review initial and comprehensive treatment plans within the required time frame. Identify and document patient care needs. Work closely with the interdisciplinary team to ensure appropriate patient care. Know, understand, maintain, and monitor patients’ rights. Provide patient/family emotional support and education. Meet in-service requirements including mandatory education. Perform other duties as required. Must read, understand, and adhere to all Physician Services policies and procedures. Practice and adhere to the Code of Conduct and Mission and Value Statement. Qualifications Post Graduate degree Minimum of 3-5 years’ experience A Clinical Mental Health Counselor license or Independent Clinical Social Worker license is required in accordance with state regulations.
