Recovery Specialist

Summary of Position Provides support services, information, education and support for clients, promotes wellness, recovery and skill building to sustain better health. Provides prevention training for managing stress and problems, making healthy lifestyle choices, recognizing and responding to relapse, managing crisis and achieving and sustaining recovery. Assists clients with personal recovery goals, helps clients develop natural support systems, and coaches clients to advocate for themselves within their healthcare systems. Implements evidence‑informed wellness interventions. Essential Duties & Responsibilities Provide group or individual training/interventions focusing on improvement of the client’s functioning and health related issues (e.g., symptom/medication management, life skills, healthy eating, weight management, smoking cessation, recognizing the dangers of alcohol and other drugs, community reintegration, etc.). Assist clients with developing their Wellness Recovery Action Plan (WRAP) and provide wellness consultation. Work in consultation/collaboration with clients, family members and other providers to implement the plan. Empower clients to self‑manage chronic health conditions and solidify wellness goals. Assist clients with motivation and overcoming barriers to goals, including access to care. Navigate health care systems and link clients to community‑based social support/services. Develop collaborative relationships with other departments, services and community health care agencies to facilitate and support wellness and quality care. Provide ongoing case management services for assigned service recipients, including assessment, planning, linking, monitoring, referral and advocacy. Provide transportation assistance to help clients achieve and maintain wellness goals and initiatives. Conduct intake for recipients entering services, explain services, gather intake information, ensure forms are understood and signed, and submit materials for processing. Conduct assessment and write assessment summaries. Assist service recipients in making discharge plans and provide follow‑up services. Knowledge, Skills & Abilities Effectively communicate via written, verbal, in‑person and virtual methods. Knowledge of the role and function of various community resources. Knowledge of and ability to instruct assigned clients in basic independent living skills. Skill in customer service/experience techniques. Skill in problem‑solving. Qualifications Education: Minimum 21 years of age per funding requirements. Associate’s degree required; bachelor’s degree in human services preferred. Education/experience equivalency may be considered upon approval. Years of Experience: Two years relevant experience working in the human services field. Required Certification/Licensure: WRAP facilitator within twelve (12) months of accepting the position is preferred. Pay Range: $16.50–$23.10 per hour.
