Key Responsibilities

• Evaluate and treat patients using medically prescribed physical therapy programs with the goal of restoring function and minimizing disability following disease or injury
• Conduct initial admission and evaluation visits, documenting clinical findings, skilled interventions, goals, and discharge plans
• Complete and submit OASIS scores to the assigned Clinical Team Manager within established timelines
• Supervise Physical Therapy Assistants as applicable
• Help clients reach their maximum performance and functional capacity within the limits of their capabilities
• Perform additional duties consistent with skills, training, and the goals of the organization

Required Qualifications

• Active Physical Therapist license in the state of California
• Minimum of one year of home health experience
• Proficiency in OASIS documentation and established reporting timelines
• Ability to travel throughout an assigned territory including Brisbane, Daly City, Colma, South San Francisco, Pacifica, and neighboring cities

Preferred Experience / Skills

• Experience supervising Physical Therapy Assistants in a home health setting
• Familiarity with home health care coordination and clinical team communication workflows
