Physical Therapist (PT)/Physical Therapy Assistant (PTA) - Home Health Per Diem

Pivot Point Home Care is hiring a per diem Physical Therapist (PT)/ Physical Therapist Assistant to provide home health visits in and around the Framingham, MA area.

This is a flexible, per-visit role for a PT or PTA who wants one-on-one patient care, control over their schedule, and the ability to work independently with support from a responsive clinical team.

This is a per diem role with flexible scheduling. Expected volume is approximately 5-15 visits per week , depending on patient census, clinician availability, and service area needs.

Compensation

PTs are paid per completed visit type:

• Start of Care: $160
• Initial Evaluation: $120
• Re-Evaluation: $80
• Routine PT Visit: $65

PTAs are paid per completed visit type:

• Standard PTA Visit: $45

Actual hourly earnings may vary based on visit type, routing, documentation time, cancellations, and scheduling. When visits are close together geographically, clinicians may be able to complete more than one visit in an hour.

Responsibilities

• Perform in-home physical therapy evaluations and visits or standard PTA visits
• Develop and update individualized treatment plans based on patient needs and goals
• Deliver skilled therapy to improve mobility, strength, balance, safety, and functional independence
• Educate patients and caregivers on exercises, fall prevention, transfer safety, and home safety
• Complete accurate and timely documentation for each visit
• Communicate patient status, changes, and concerns with the care team
• Collaborate with and supervise PTAs as clinically appropriate
• Participate in team communication and required in-service education as needed

Requirements

• Current Massachusetts Physical Therapist or PTA license
• At least 1 year of physical therapy or PTA experience
• Home health experience preferred
• Reliable transportation for patient visits
• Ability to complete visits in and around Framingham, MA
• Strong clinical judgment, documentation, and communication skills
• Ability to work independently in the field
• Compassionate, patient-centered approach
• Successful completion of required background check and onboarding requirements
