St. Croix Hospice in Missouri is seeking a Registered Nurse Case Manager to lead each patient’s hospice care team and develop the Plan of Care with the Hospice Medical Director, patient, family and facility staff. The RN Case Manager provides day-to-day patient care, manages medications, maintains records and coordinates with physicians and interdisciplinary teams to meet changing needs, prioritizing comfort and quality of life in home settings. #J-18808-Ljbffr
