Charge Nurse (LVN/RN) - 10P-6A Shift

Job Summary:

We are seeking a highly skilled and compassionate Charge Nurse (LVN/RN) to lead our interdisciplinary team at Brentwood Place Three, a leading skilled nursing and rehabilitation center in Dallas, Texas. As a Charge Nurse, you will be responsible for overseeing the care and services provided to our patients, ensuring that they receive the highest quality of care and support.

About the Role:

• Lead a team of dedicated healthcare professionals to provide exceptional patient care and services
• Develop and implement individualized care plans that address the unique needs of each patient
• Collaborate with other departments, such as physical therapy, occupational therapy, and speech therapy, to ensure comprehensive care
• Monitor patient condition and progress, adapting care plans as needed
• Provide education and guidance to patients and their families on disease management, self-care, and discharge planning
• Ensure compliance with policies, procedures, and regulatory requirements

Responsibilities:

• Oversee and coordinate the daily activities and operations of the nursing team
• Provide direct patient care, including administering medications, performing treatments, and assessing patient condition
• Conduct initial and ongoing assessments of patients, including physical, emotional, and social evaluations
• Develop and implement discharge plans that promote patient safety and independence
• Participate in quality improvement initiatives to improve patient outcomes and satisfaction
• Maintain accurate and detailed records of patient care and progress
