Overview The Health and Wellbeing Coach provides personalised support to individuals to empower them to take control of their own health and wellbeing with a focus on lifestyle behaviour change. Core areas include: Healthy Eating and Nutrition Smoking Cessation Physical Activity Weight Management Alcohol consumption NHS Health Checks (outreach) Responsibilities Facilitate people in establishing and attaining health‑promoting goals set by the person to change lifestyle‑related behaviours, aiming to reduce health risks, improve self‑management of chronic conditions, and enhance quality of life. Deliver face‑to‑face, telephonic, and digital health coaching support to individuals from a wide range of community and healthcare settings through one‑to‑one and group‑based support as appropriate. Provide a health‑coaching service for eligible participants, supporting them to gain and use knowledge, skills, and confidence to become active participants and reach their self‑identified goals. Key tasks include: Triage and assessment Agenda setting Lifestyle behaviour change support (e.g., smoking cessation, increased movement, healthy diet, reduced alcohol consumption) Problem solving Motivational interviewing Goal setting Signposting Advocating the use of behaviour‑tracking tools Daily caseload appraisal to ensure appropriate intensity and frequency of support Using a strength‑based approach and maximizing community assets and the wider health and wellbeing economy Ensure delivery aligns with quality standards, national and government guidelines, and role boundaries. Identify when coaching is not appropriate and sensitively discuss alternatives or appropriate referrals. Support outreach and community engagement work, including: Screening services (e.g., NHS Health Checks) Community and stakeholder events External meetings and networks Roadshows and pop‑ups Adhere to business and contract performance requirements such as: Implementing Standard Operating Procedures (SOP) Fulfilling policies and quality‑management framework expectations (e.g., data protection, information security, health and safety, safeguarding) Participating in audits and performance observations, and pursuing continuous professional development Proactive supervision and training commitment, achieving quality standards, high performance, and Key Performance Indicators (KPIs) Maintain accurate reporting and data collection to manage caseloads efficiently, support population behaviour change, enhance care coordination, ensure single reporting point for participants, and capture activity, productivity, utilisation, and outcomes. Qualifications & Experience Essential Relevant health‑coaching qualification or an accredited health‑coaching skills programme A minimum of six months of direct health‑coaching delivery Experience delivering behaviour‑change interventions for specific lifestyle issues (e.g., smoking cessation, physical activity, NHS Health Checks, weight management, alcohol reduction) Evidence of supporting vulnerable individuals through a change process Experience of caseload management via a Case Management System Experience of inputting and processing data accurately against targets and deadlines Evidence of continuous professional development to maintain up‑to‑date coaching provision Experience working in a health, community, or social care environment, or good understanding of maintaining a healthy lifestyle Desirable Membership of a professional body (ICF, EMCC, AoC, UKHCA) Experience managing and supervising individuals and teams Degree level education in a relevant subject (e.g., Psychology, Behavioural Science) Experience supporting people remotely/telephonically/digitally Evidence of working with individuals with long‑term health conditions Individual Competencies Essential Strong understanding of the social determinants of health Strong understanding of population‑based approaches including segmentation and risk stratification Strong understanding of behaviour‑change principles and methodology (e.g., COM‑B model, behaviour change wheel) Excellent oral and written communication skills with diverse backgrounds Demonstrable core skills and competencies such as: Selecting and applying a range of health‑coaching models, conversation frames, and techniques Detailed knowledge of personalisation, patient activation, health behaviour change, self‑efficacy, intrinsic motivation, and asset‑based approaches Detailed understanding of self‑management support and associated techniques Advanced skills development through practice, reflection, and planning for application to practice Excellent internal and external stakeholder engagement and management Good understanding of confidentiality and safeguarding principles Strong written and verbal communication skills for effective case management Strong track record of interpersonal relationships and building, developing, and maintaining them Effective caseload management with accurate data collection, entry, timely recording, and delivery against SLAs and SOPs Confidence in using evolving digital technologies to support people through behaviour‑change processes Ability to manage time independently, effectively, and meet deadlines Effective safe and sensitive data management aligned with information security standards Excellent Microsoft Office skills and ability to learn new systems quickly Commitment to personal development and training Commitment to flexible working (including evenings and weekends) to be available to all residents, shift workers, and those with children Commitment to supporting overall team/contract performance to the highest standards, undertaking duties in line with business requirements Desirable Experience delivering health screening services (e.g., NHS Health Check) Effective delivery of programmes in line with contractual requirements and SLAs Key Business Priorities Internal Co‑workers, managers, and wider team Health Division colleagues Maximus Central Division Maximus companies and associates Colleague forums External Local Authority Integrated Care Partnerships / Boards Community and Voluntary sector Population being served / supported Sub‑contractors and key partners Community stakeholders Co‑location cooperatives Venue providers Healthcare settings including GP Practices / Primary Care Networks Salary Minimum Salary: £25,311.00; Maximum Salary: £28,700.00 EEO Statement MAXIMUS is committed to developing, maintaining and supporting a culture of diversity, equity and inclusion throughout the recruitment process.
