Pride Health is hiring a RN - Home Health to support our client's healthcare facility in Denver, CO.

Job Details:

Schedule: Monday-Friday | 8:00 AM-4:30 PM | On-Call Rotation | 40 Hours/Week

Location: Denver, CO

Coverage areas include the Denver Metro Area

Mileage reimbursed: Yes

Job Type: Contract

Contract Length: 14 Week/s

Pay Range: $52-$54

*Pay offered is based on experience, expertise, credentialing, and education.

Duties:

• Provide wound care and manage patient needs as an RN Case Manager.

• Complete Start of Care OASIS assessments and develop individualized plans of care.

• Perform labs, medication management, and catheter management.

• Provide patient education and coordinate care with physicians and interdisciplinary healthcare teams.

• Complete accurate and timely patient documentation and follow-up.

Requirements:

• License(s): Active RN license

• Certification(s): BLS

• Years of experience: 1+

• Skills/Specialties/Technology: Home Health experience preferred; OASIS and wound care experience preferred
