Experience: 1 year
Licenses: OT-IL(Copy needed)
Certifications: OTR-National Board for Certification in Occupational Therapy(Copy needed); BLS-AHA(Copy needed)

DOB Required
Work History: Should not be more than 90 days of gap in last 1 year Position is for NICU/PEDS, so NICU/PEDS experience is required! MUST have IL license in hand
