Youth Transition Navigator: Empowerment & Support
The New Foster Care
The New Foster Care in Michigan is seeking a Transition Navigator to partner with youth and families, providing trauma-informed, person-centered case management across five transition domains to support personal, educational, and career goals. Working under the Program Director and Transition Services Supervisors, you will maintain a caseload, develop individualized plans, coordinate across systems, and travel throughout the Metro Detroit area to meet participants where they are. #J-18808-Ljbffr The New Foster Care
