Job Description
Title: Registered Nurse - Home Health

Schedule: 8 hours per shift

Assignment Details:
- Contract Length: 14 weeks
- Guaranteed Hours: 40 hours per week

Requirements:
- Current state license as a Registered Nurse
- Homecare Homebase Software Experience
- OASIS Experience
- 1-2 years experience as a Registered Nurse in a clinical care setting
- 2+ years Home health experience
- Proof of current CPR certification
- Current driver's license with reliable transportation and insurance

Roles & Responsibilities:
- Assess patients in home health or hospice settings to determine their needs and develop a comprehensive Plan of Care.
- Coordinate with the interdisciplinary team, patients, and families to ensure continuity of care.
- Document patient visits, evaluations, and changes in care promptly and accurately.
- Educate patients and families on care plans, medications, and home safety, promoting self-care practices.
- Supervise and provide clinical direction to auxiliary nursing staff.
- Arrange referrals to other agencies when necessary.
