Job Title: Clinical Nurse Profession: RN Specialty: Outpatient Duration: 9 months Shift: 8AM - 4PM Hours per Shift: 40 Experience: Minimum 2 years of recent hospital outpatient RN experience required License: NYS RN License Certifications: BLS, ACLS Must-Have: - Ambulatory Care Nursing experience - Patient Care Management - Critical Thinking - Patient Assessment - Interdisciplinary Collaboration Description: The Clinical Nurse is a Registered Professional Nurse who provides safe and competent quality care. Care is based on nursing theory and research for a designated group of patients and significant others. Delivers patient care by integrating the tenets of the professional practice model of Relationship Centered Care. Utilizes the nursing process based upon theory, research, evidence-based practice, and approved organizational standards. Assesses and evaluates patient care needs while applying critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in nursing care delivery. Collaborates with the interdisciplinary healthcare team for the development, implementation, and evaluation of the plan of care. Prioritizes all aspects of patient care, including teaching, rounding, coaching, and planning after hospital care. Educates patients and caregivers about their plan of care, transition of care, health promotion, and disease prevention. Communicates effectively and professionally with patients, families, and healthcare team members. Manages assignments within the modified primary nursing care delivery model. Role models the tenets of Relationship Centered Care. Facilitates actions such as patient admissions, bedside shift reports, purposeful hourly rounding, and discharge wrap-ups. Considers patient values, preferences, cultural diversity, and expressed needs in all aspects of care. Establishes and maintains therapeutic relationships with patients and families. Uses evidence-based practices to enhance understanding of patient perceptions of care. Initiates service recovery for patients and family members with concerns. Demonstrates caring, respect, compassion, empathy, and active listening. Conducts purposeful hourly rounding on assigned patients. Practices safe and efficient patient-centered care. Identifies and escalates barriers and opportunities for improvement. Demonstrates clinical skills and knowledge crucial for quality and safety. Implements process improvement strategies based on evidence-based procedures. Collaborates with interdisciplinary teams for quality improvement projects and nursing research studies. Contributes to quality and safety practices and compliance measures. Utilizes documentation to support quality and performance improvement initiatives. Performs charge nurse duties as assigned. Coordinates with management to ensure supplies are utilized appropriately. Escalates resource deficits to relevant personnel. Documents care accurately to comply with regulatory and legal standards. Facilitates patient throughput during shifts. Contributes to an environment of care that supports clinical colleagues and patients. Advances clinical competence to progress from novice to expert. Communicates with styles that demonstrate caring and respect. Incorporates ethical principles into decision-making for patients and families. Encourages a spirit of scholarship, inquiry, and life-long learning. Participates in decision-making for specialty practice councils and unit initiatives.
