Registered Nurse - Hospice

Job Description

Job Summary:
Seeking a compassionate and experienced RN for a hospice assignment, providing high-quality patient care in a home health setting. The role involves managing a caseload, performing home visits, and coordinating care within a designated territory.

Responsibilities:

• Provide comprehensive end-of-life care to hospice patients in their homes.
• Conduct patient assessments, develop care plans, and deliver treatments aligned with hospice protocols.
• Document all clinical activities accurately within specified timeframes using EMR systems.
• Coordinate with families, caregivers, and interdisciplinary teams to ensure optimal patient comfort and support.
• Travel up to 75 miles daily to patient homes, managing an average caseload of 5-8 patients per day.
• Attend periodic office meetings and participate in on-call rotations as required.

Qualifications:

• Hold an active RN license in good standing.
• Have a minimum of one full year of recent adult home health or hospice experience.
• Be proficient in Homecare Homebase (HCHB) and familiar with OASIS documentation.
• Possess current CPR and BLS certifications.
• Have reliable transportation, a current driver's license, and liability insurance.
• Demonstrate strong communication skills and ability to manage travel logistics effectively.
