Case Manager – Acute Rehab

Shift: Monday–Friday, 8:00 AM–4:30 PM
Hours: 40/week
Unit: 60-bed acute rehab hospital
Ratio: Approx. 1:15
EMR: UDS Pro, Meditech, Pinpoint
Required: Healthcare professional licensure with case management experience
Preferred: CCM, ACM, RN-BC, or rehab nursing certification
Experience: 2–3+ years preferred
Skills: Utilization review, discharge planning, family/team conferences, strong communication, and basic computer skills
