HARBOR HEALTHCARE SYSTEM
Harbor Healthcare System, based in Austin, TX, is seeking a qualified part-time RN Case Manager for outpatient care. The RN-Case Manager works as part of the interdisciplinary care team, coordinating admissions, assessments, discharges and care planning to ensure timely and appropriate patient and family support.
The ideal candidate holds a Texas RN license, has 1+ year RN experience (Hospice or Home Health preferred), excellent written and verbal communication, and reliable transportation to
