Short-term rehabilitation: For patients recovering from surgery, illness, or injury. Collaborative care: An interdisciplinary approach involving nursing, therapy, and medical staff working in unison. Position Summary The Physical Therapist (PT) delivers direct rehabilitation services in physical therapy to patients referred by physicians, adhering to all relevant laws, regulations, and organizational standards. Qualifications and Experience Graduation from an accredited physical therapy program (BSPT, MSPT, or DPT; those graduating in 2003 or later must hold an MSPT or DPT). Current registration/licensure in the applicable state, with an active license in good standing throughout employment. Preferred one year of experience; however, new graduates are welcome to apply. CPR certification required upon hire or to be obtained during orientation, with ongoing maintenance of certification during employment. Specific Job Responsibilities Engage in community health initiatives as appropriate. Proficient in Microsoft Word, Excel, and email communication. Collaborate with patients, families, and support departments to effectively address patient needs. Exhibit proper body mechanics consistently. Make independent decisions when necessary. Understand and comply with practices, procedures, laws, and regulations governing post-acute care facilities. Implement and interpret departmental programs, goals, objectives, policies, and procedures. Demonstrate proficiency in all competency areas, including patient rights, safety, and sanitation. Maintain professional relationships with colleagues, vendors, and others. Safeguard the confidentiality of proprietary and sensitive information. Adhere to organizational policies, including those related to harassment and compliance. Exhibit integrity and professionalism by following the organization’s Code of Conduct and completing mandatory compliance training. Essential Functions Implement physical therapy treatment plans for patients. Set, evaluate, and adjust realistic, measurable, and functional goals. Supervise and assess the care provided by PTAs, Rehab Aides, and PT and PTA students. Document care accurately and in a timely manner. Utilize therapy software effectively and accurately. Provide excellent customer service and maintain a positive attitude towards patients. Assist in the safe evacuation of patients when necessary. Demonstrate reliable attendance and punctuality. Employ reasoning skills and sound judgment in decision-making. Communicate effectively and collaborate within an interdisciplinary team. Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving throughout working hours.
