Position Title: Patient Navigator Location: Syracuse, NY Department: Neurosurgery 113 Employment: Full‑time, Days, 8:00 AM – 4:30 PM Pay Range: $62,353.20 – $79,040.00 (DOE) Job Code: 32095 Posted: 04/02/2026 Position Description The Patient Navigator is a key member of the practice who will provide a single and centralized point of patient access to assist clinicians and patients in managing specialty and diagnostic referrals. When necessary, the Navigator will lead patients through the health care system from the initiation of the referral, to obtaining prior authorization, scheduling appointments, and tracking and following up on results. Responsibilities Act as a single point of patient access for specialty and diagnostic referrals. Assist clinicians and patients with referral initiation, prior authorization, scheduling, and result tracking. Provide clear communication and follow‑up to ensure timely completion of required steps. Maintain accurate documentation and files related to patient referrals. Collaborate with physicians to individualize and enhance patient care. Required Qualifications Registered Nurse. Experience in a multi‑specialty physician office. Excellent customer service skills. Strong computer skills. Excellent interpersonal and intra‑professional relations.
