Social Worker Responsibilities Plan, coordinate, and implement behavior intervention plans to help clients, their families, and care teams thrive in their environment. Interview clients and stakeholders, assess current condition, needs, strengths, and weaknesses. Provide services that address various client and family needs through setting tailored measurable goals. Work with children and adults with disabilities in home, residential, and community settings. Manage and implement behavior intervention plans to help clients with disabilities in a variety of settings. Address each case as a unit and set tailored measurable goals. Monitor and evaluate client progress and modify treatment plans accordingly. Offer information and counseling on best course of action during sessions. Maintain accurate records and report on client status. Act as a resource and cooperate with multidisciplinary teams. Refer clients to community services. Requirements and Skills Working knowledge of social theories and practices. Social perceptiveness and empathy. Ability to build and maintain professional helping relationships. Ability to relate and communicate with diverse populations and groups. Resilience and ability to assess situations. Willingness to submit to background checks.
