Job Title: Care Manager, Hospice RN Profession: Nursing Specialty: Care Manager, Hospice RN Duration: 16 weeks Shift: Days: Monday to Friday with some weekend coverage Hours per Shift: 36 Experience: Minimum 1 year License: Registered Nurse License Certifications: OASIS experience, home health experience Must-Have: OASIS experience is a must. Home health experience is required. Home care traveler experienced in OASIS is necessary for case management. This position requires a rapid response for crisis situations. Holiday coverage is required.
