Responsibilities:
- Pass medications on assigned shift.
- Monitor medication administration to ensure accuracy, compliance, and resident safety.
- Conduct routine medication and treatment storage reviews in accordance with policy and procedure.
- Educate residents and caregivers on proper medication use, potential side effects, and adherence to treatment plans.
- Collaborate with healthcare providers, including physicians, pharmacists, and other nurses, to optimize medication therapies.
- Maintain accurate and detailed medication records and documentation.
- Other duties as assigned by Health Care Director

Requirements:
- Required: Active Licensed Practical Nurse (LPN) license in Pennsylvania.
- Required: Minimum of 2 years of clinical experience in a healthcare setting (focus on medication management preferred).
- Required: Strong understanding of pharmacology, medication administration, and drug interactions.
- Required: Excellent communication and interpersonal skills.
- Required: Ability to work independently and as part of a multidisciplinary team.
- Required: Proficiency in electronic medication administration records (eMAR) systems.
Preferred Qualifications:
- Prefer: Experience in personal care, long-term care, hospice, or home health settings.
- Prefer: Knowledge of regulatory standards and guidelines related to medication management.
- Prefer: Commitment to continuous learning and professional development.
