Join Our Caring CommunityAs a CenterWell Home Health nurse, reporting to the Clinical Manager, you will provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you'll manage care plans that support recovery and help patients get back to the life they love.As a Home Health LVN, you will:Provide skilled nursing care to patients, working onsite in their homes, under the direction of an RN.Implement individualized care plans in collaboration with the patient, family, and healthcare team.Educate patients and their families on disease management, treatment options, and self-care techniques.Maintain accurate records of patient care, including documentation of patient visit, interventions, and outcomes.Maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team promptly regarding patient or family needs and status changes. Participate in care coordination activities and discharge planning.Use Your Skills to Make an ImpactRequired Qualifications:Current nursing license in the practicing stateValid drivers license, auto insurance and reliable transportationCurrent CPR certificationTwo years experience as an LPN/LVN in a clinical settingPreferred Qualifications:Nursing experience in a Home Health or Hospice settingPay Per Visit/Unit Rate $33.00 - $47.00Additional InformationTB Statement: This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.Driving Statement: This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance.
