Description:
The Direct Service Provider (DSP) delivers person-centered Home and Community-Based Services (HCBS) under the Kentucky 1915(i) RISE Initiative. The DSP is responsible for implementing the specific services identified in the participant’s Person-Centered Service Plan (PCSP), focusing on skill development, community integration, and health and safety monitoring.

Qualifications:
Education & Experience:
High School Diploma or GED required.
Relevant experience in human services, mental health, or foster care preferred.
Successful completion of all agency-required screenings and trainings.
