Promote continuity of care and cost effectiveness through whole-person in-patient and out-patient management. Coordinate care integration, case management, utilization review, and care planning for assigned members. Collaborate with multidisciplinary teams to support member engagement, adherence, and care transitions. Document care plans, assessments, and outcomes in accordance with program standards and regulatory requirements. Support quality improvement initiatives and monitor care outcomes within the care management framework. Requirements RN with responsibilities centered on health plan care management and case management activities. Experience or exposure to inpatient and outpatient care management, utilization review, and care planning. Ability to coordinate across multidisciplinary teams to support member-centered care. Strong communication and documentation skills to support member engagement and care transitions.
