Location: 1355 Clayton Rd, San Jose, CA 95127, USA As a Registered Nurse (RN) at THE RIDGE POST-ACUTE i you will play a critical role in delivering high-quality nursing care to post-acute patients recovering from illness, surgery, or injury. Your primary focus will be on assessing patient conditions, developing and implementing individualized care plans, and monitoring patient progress to ensure optimal recovery outcomes. You will collaborate closely with interdisciplinary healthcare teams, including physicians, therapists, and support staff, to coordinate comprehensive care. Additionally, you will provide education and support to patients and their families to promote health management and prevent complications. Your expertise and compassionate care will directly impact patient satisfaction and the overall success of the facility’s rehabilitation and recovery programs. Minimum Qualifications: Current and valid Registered Nurse (RN) license in the state of employment. Graduation from an accredited nursing program (Associate's or Bachelor's degree in Nursing). Basic Life Support (BLS) certification. Minimum of one year of clinical nursing experience, preferably in post-acute, rehabilitation, or long-term care settings. Strong knowledge of nursing principles, patient care standards, and medical terminology. Preferred Qualifications: Bachelor of Science in Nursing (BSN) degree. Advanced certifications such as Certified Rehabilitation Registered Nurse (CRRN). Experience with electronic health record (EHR) systems and documentation software. Familiarity with state and federal healthcare regulations related to post-acute care. Demonstrated leadership or charge nurse experience. Responsibilities: Conduct thorough patient assessments and document medical histories, symptoms, and vital signs accurately. Develop, implement, and update individualized nursing care plans based on patient needs and physician directives. Administer medications and treatments safely and monitor patients for adverse reactions or changes in condition. Collaborate with interdisciplinary teams to coordinate patient care and participate in care planning meetings. Educate patients and their families on post-acute care procedures, medication management, and health maintenance. Maintain compliance with healthcare regulations, facility policies, and quality standards. Respond promptly to patient emergencies and provide appropriate interventions. Supervise and mentor licensed practical nurses (LPNs) and nursing assistants as needed. The required skills enable the RN to perform comprehensive patient assessments, develop effective care plans, and administer treatments safely, ensuring patient safety and recovery. Strong communication skills are essential for educating patients and collaborating with multidisciplinary teams to deliver coordinated care. Critical thinking and problem-solving abilities are used daily to respond to changing patient conditions and emergencies promptly. Proficiency with electronic health records facilitates accurate documentation and efficient information sharing among care providers.
