Physical Therapist - Acute Care
Preferred Home Health Care & Nursing Services
Role: The physical therapist evaluates, plans, and implements rehabilitative programs to improve patient mobility, reduce pain, and prevent deformity due to illness or injury. Responsibilities: Develop and revise treatment plans with patient and family input, provide education, discharge instructions, and referrals. Collaborate with physicians on treatment programs, establish and evaluate care plans, and document sessions accurately. Qualifications: Bachelor's, Master's, or Doctorate in Physical Therapy; valid PT license; BLS certification. Skills: Effective communication, customer service, teamwork, basic computer skills, ethical conduct, confidentiality, and adherence to safety and infection control standards. Additional Duties: Participate in professional development, mandatory training, and team activities.
