Mobile Nurse Practitioner - Primary Care | San Fernando & Antelope Valleys

As a Mobile Nurse Practitioner, you’ll provide comprehensive primary care to medically complex patients in Board & Care Homes, Assisted Living Facilities, and Skilled Nursing Facilities throughout the San Fernando Valley and the Antelope Valley.

This is an ideal opportunity for an NP who values autonomy, relationship-based care, and practicing at the top of their license . You’ll work alongside an interdisciplinary team committed to improving outcomes for vulnerable and underserved patients.

Compensation & Benefits:

• $120,000–$178,000 base salary + performance bonus

Qualifications:

• MSN from an accredited program and current NP certification
• At least 1 year of NP or equivalent advanced practice experience
• Active, unrestricted California RN and NP licenses
• Ability to travel throughout assigned area
