Job Description

Provides direct patient care full time and works as a member of an expanded healthcare team (social worker, dietician, clinical pharmacists, behavioral health clinician, and a care manager) to oversee and coordinate medical care for a population of Veterans in a group practice setting utilizing the PACT model.

Performs initial histories and physical examinations on outpatients using CVT technology and/or face-to-face visits.

Provides ongoing outpatient care for assigned patients.

Screens outpatients to determine the need for further care.

Prescribes medications within defined scope of practice.

Orders diagnostic studies such as laboratory tests, x-rays, electrocardiogram.

Reviews need for and orders health promotion and disease prevention measures.

Initiates, responds to and expediates requests for consultations, special tests and studies

Provides education and counseling of patients and families in preventive care, medical conditions, and use of prescribed treatments and drugs.

Attends staff meetings, team meetings, some committee meetings, and completes some peer review evaluations

Provides education and guidance to Medical Residents in managing medical care to Veterans in the outpatient setting

Work Schedule: 8:00am-4:30pm Monday- Friday
