Position: Home Health Physical Therapist

In this role, you will:

• Design and implement prescribed physical therapy treatments for patients in their homes, aiming for the highest level of physical function.
• Evaluate and document patients' physical capabilities to assist in physician assessments and inform treatment objectives.
• Create and execute a rehabilitation program aligned with the physician's treatment plan and the goals of the patient care team.
• Provide clear written care instructions to Physical Therapy Assistants (PTAs) and Home Health Aides, supervising their performance and documentation.
• Thoroughly document patient observations, interventions, and evaluations in a timely manner.
• Communicate patient progress to the physician, Clinical Manager, care team, patient, and family, while submitting all necessary evaluations, treatment plans, progress notes, and discharge summaries.

Utilize Your Skills to Make a DifferenceRequired Qualifications:

• Degree from an accredited Physical Therapy Program recognized by the APTA.
• Current and unrestricted Physical Therapy license.
• Valid CPR certification.
• Experience working collaboratively with a healthcare team.

Preferred Qualifications:

• At least one year of experience as a Physical Therapist.

Compensation Details

Pay Range:

• $59.00 - $83.00 per visit/unit.
• $93,000 - $128,000 annual base salary.

Additional Information

TB Screening: This position involves direct patient interaction and is part of our Tuberculosis (TB) screening program. If selected, you will need to undergo TB screening.

Driving Requirements: This role is included in our driver safety program, necessitating a valid state driver's license and personal vehicle liability insurance. You must maintain vehicle insurance that meets or exceeds state minimum requirements, or at least $25,000 for bodily injury per person, $25,000 for bodily injury per event, and $10,000 for property damage, whichever is higher.

40 hours per week.

Compensation Range

The salary range provided reflects a good faith estimate of the starting base pay for full-time (40 hours per week) employment at the time of posting. Actual pay may vary based on geographic location and individual qualifications, including skills, experience, education, and certifications.
