Population Health Coordinator

Job Role Population Health Coordinator, reports to Manager of Population Health. The role supports value‑based care and population health initiatives, including data analysis, documentation, and collaboration with providers and health plans. Responsibilities Collect data and patient records to close HEDIS care gaps and maintain STAR Ratings Meet with multiple health plan representatives related to their incentive programs Work with Provider Portals to close care gaps Attest to patient's historical diagnosed chronic and suspect conditions Assist Manager with participation in population health programs Qualifications 1 year experience in Population Health or Medical Billing Minimum 1.5 years experience in a Corporate / Health care Setting MA experience highly preferred but not required Ability to work independently as well as in a team Full‑time, in‑office presence required High level of professionalism Excellent written and verbal communication skills Excellent organizational and problem‑solving abilities Ability to work collaboratively with diverse teams and departments Knowledge of healthcare regulations and compliance standards Strong communication and interpersonal skills Proficiency in electronic health records (EHR); eCW preferred, Microsoft products, and other relevant software Must have HS diploma or GED equivalent Benefits Career advancement opportunities Leadership and mentoring Resources to further career Fun work environment (events, holiday parties) Medical/vision/dental/401(k)/PTO/paid holidays Supportive and positive work environment #J-18808-Ljbffr Optima Medical
