LPNTryfacta is seeking a Licensed Practical Nurse (LPN) for our client in Florence, WI 54121. This is a 6 weeks assignment. Auto offerResponsibilities for this position include, but are not limited to:Provide direct nursing care to residents in a long-term care/skilled nursing setting during the night shift.Administer medications, treatments, and therapies as prescribed while monitoring resident responses.Perform resident assessments, monitor vital signs, and promptly report changes in condition to the RN or provider.Assist with activities of daily living (ADLs), mobility, transfers, and resident comfort while promoting dignity and independence.Maintain accurate documentation of nursing care, medications, and resident status in PointClickCare (PCC).Collaborate with physicians, RNs, CNAs, therapists, and other interdisciplinary team members to provide quality resident care.Ensure compliance with infection prevention, resident safety, and facility policies and procedures.Respond appropriately to emergencies and provide compassionate, resident-centered care throughout the shift.Active Wisconsin LPN License or Compact LPN License (if applicable).To be considered for this position, you should have:Minimum 1 year of recent Long-Term Care (LTC) or Skilled Nursing Facility (SNF) experience preferred.Current BLS Certification preferred.Experience with PointClickCare (PCC) or similar electronic medical record (EMR) system preferred.Strong skills in medication administration, patient assessment, wound care, and documentation.Excellent communication, teamwork, and time management skills.Ability to work Night Shift (10:00 PM – 6:30 AM).48 guaranteed hours per week with overtime available at the facility's discretion.Tryfacta is an Equal Opportunity/Affirmative Action Employer.
