Title: Social Worker, MSW New Grad Location: Fountain Valley Department: Social Work Services Status: Per-diem Shift: Day Pay Range*: $40/hour Position Summary This position requires full understanding and active participation in fulfilling the mission of Orange Coast Memorial Medical Center. The Social Worker (New Grad) provides social work services to patients and families in all hospital areas specific to assignment. The role involves addressing psycho-social issues related to acute or chronic illness or disability, intervening to remove barriers to safe and timely transition, and advocating for patient self‑determination. The Social Worker also focuses on ensuring quality care while engaging in cost‑effective measures such as Performance Improvement Projects, LEAN activities, and other hospital‑wide initiatives. Professional services include, but are not limited to, biopsychosocial assessment, brief treatment, crisis intervention, patient education, discharge planning, custodial placements, and resource management. Essential Functions and Responsibilities of the Job Conduct psycho‑social assessments and interventions with patients and families Address psycho‑social issues and concerns throughout the continuum of care Navigate systems and advocate to assist patients in accessing appropriate resources and services Make mandated protective services reports based on psycho‑social assessment of patient and family Address barriers to a safe and timely discharge Be present at work and be on time; required to be on campus during scheduled core business hours to accommodate crisis issues and support the medical team, peers, and staff assigned to a specific area Follow company policies, procedures, and directives Interact in a positive and constructive manner Prioritize and multitask Perform other duties as assigned Minimum Requirements Qualifications/Work Experience 1 year or less of direct social work experience in an acute healthcare setting preferred Competency is defined and measured via the Social Worker Job Specific Competency Assessment and Checklist; each Social Worker, MSW (New Grad) is assessed at orientation and annually using these tools Education/Licensure/Certification Master of Social Work (MSW) Degree from an accredited School of Social Work required Current American Heart Association Basic Life Support Card #J-18808-Ljbffr MemorialCare
