Social Worker - PRN

Providence Care is seeking a compassionate and experienced PRN Social Worker to support hospice patients and their families.In this role, you will provide emotional support, resource coordination, and guidance through end-of-life care; working closely with an interdisciplinary team to ensure patients receive care with dignity and respect.What You Will DoComplete psychosocial assessments for patients and familiesProvide counseling and emotional supportAssist with care planning alongside the clinical teamConnect families with community resources and servicesSupport bereavement needs as appropriateEducate patients and families on available services and optionsMaintain accurate and timely documentationRequirementsBachelor’s or Master’s Degree in Social Work (CSWE accredited)Active South Carolina license (LMSW, or LISW-CP)Minimum of 1 year experience in a healthcare settingHospice or end-of-life experience preferredStrong communication and interpersonal skillsAbility to work independently and manage time effectivelyProvidence Care is an Equal Opportunity Employer.
