Pairing at-home screening kits with synchronous video visits by licensed Nurse Practitioners, Reperio provides patients a complete picture of their cardiovascular and metabolic health without leaving home. Our NPs review same-day lab results alongside exchange-pulled medical history, lifestyle questionnaires, and mental health screeners to deliver personalized, evidence-based counseling and care coordination. Position Overview Reperio Health is seeking experienced, independent Nurse Practitioners (FNP or AGNP) to join our growing virtual care team on a per diem or part-time 1099 basis. In this role, you will conduct synchronous video visits, interpret at-home screening results in clinical context, and deliver meaningful preventive counseling to patients across the country. This is a non-prescribing, non-medication-management role. Our NPs are experts in screening interpretation, patient education, and care coordination. This is all about empowering patients to take action and connecting them to the right resources and providers when needed. What you’ll do on every visit Review and interpret at-home screening results: blood pressure, lipid panel (HDL, LDL, triglycerides), glucose, weight, BMI, waist circumference, and relative fat mass Administer and discuss mental health screeners (PHQ-9, GAD-7), provide brief supportive counseling, and complete suicidality safety checks when indicated (PHQ-9 item 9 positive), with warm handoff to crisis resources or behavioral health Deliver plain-language, patient-centered counseling on cardiovascular and metabolic risk, nutrition (DASH/Mediterranean), physical activity, weight and body composition, tobacco and alcohol cessation, and sleep hygiene Apply USPSTF and CDC/ACIP preventive screening and immunization guidelines appropriate to the patient’s age and sex Coordinate care: assist unattached patients in finding a PCP, issue expedited or same-day PCP/urgent care referrals for red flags, recommend specialist referrals (cardiology, endocrinology, behavioral health, sleep, dietitian, smoking cessation), and escalate to ED or 911 for emergencies What is not in scope ReperioCare clinicians are not a substitute for a patient’s primary care provider. This role does not include prescribing medications, adjusting existing prescriptions, performing medication management, or ordering treatment. The visit is focused entirely on screening, interpretation, counseling, and care coordination. Requirements Family Nurse Practitioner (FNP) or Adult-Gerontology NP (AGNP) certification Active NP licensure in 7 or more states; multi-state compact participation strongly preferred Full practice authority and independent prescribing in all licensed states Minimum 3 years of NP experience in a primary care setting (Family Medicine, Internal Medicine, or similar) Minimum 1 year of virtual care / telehealth experience with synchronous video or phone delivery Strong background in preventive care, metabolic health, or chronic disease management Demonstrated experience with patient education and preventive counseling Tech-literate and comfortable navigating EHRs, digital health platforms, and evolving clinical tools Available for per diem, part-time, or flexible engagement on a 1099 basis Preferred Experience in a startup, early-stage, or high-growth healthcare environment Proficiency with AI-assisted clinical documentation tools (e.g., Ambience, Abridge, Nabla, or similar) Background in care coordination, population health, or value-based care models Familiarity with remote patient monitoring or digital therapeutics platforms Experience working across multiple clinical specialties in a virtual setting Compensation $90.00 - $120.00 per hour on average Time-based plan rewarding complexity over speed Interview Process Virtual pre-screen: A brief 15–20 minute call with Kyle or Matt to review your background, licensure footprint, and availability.
