Pay Range:$70,000-$75,000 Per Year Description
We are looking for a compassionate Geriatric Care Manager/ Licensed Master Social Worker specializing in geriatric care to join our client's team.
This full-time position is based in New York, NY, with a Monday to Friday schedule.
The role involves working in the field and collaborating with a supportive team, requiring proficiency in general office software and electronic documentation systems. Requirements
Conduct regular home visits in Manhattan, Brooklyn and Queens.
Possess a valid driver's license and reliable transportation.
Attend weekly in-person team meetings at our office.
Maintain flexibility to adjust schedules based on client needs.
Be comfortable working independently and as part of a team.
Strong organizational and time-management skills.
Excellent interpersonal and communication skills. Qualifications
LMSW or LCSW required.
Experience in working with older adults and care management.
Knowledge of Medicare, Medicaid, Social Security, and community services.
