Description

• Assess patients in inpatient or outpatient settings and develop customized treatment plans tailored to their needs
• Implement targeted physical therapy techniques that enhance mobility, strength, balance, and overall function
• Engage with a team of nurses, physicians, case managers, and other rehabilitation professionals to achieve optimal patient outcomes
• Educate patients and their families about therapeutic goals, discharge plans, and home exercise routines
• Maintain accurate documentation of evaluations, treatments, and patient progress per facility and regulatory requirements
• Participate in multidisciplinary care team meetings and conferences to ensure streamlined and effective care delivery

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification is preferred
