The position runs on a daytime schedule, five days a week, 08:00 to 16:30, in a long-term care setting focused on rehabilitation and resident function.

Responsibilities
- Conduct initial evaluations and develop individualized plans of care for residents requiring physical therapy.
- Implement and document treatment programs focusing on mobility, strength, balance, gait, and activities of daily living.
- Monitor progress, reassess goals, and adjust plans of care to optimize functional outcomes.
- Collaborate with an interdisciplinary team including nursing, therapy assistants, and physicians; participate in care planning and daily rounds.
- Ensure patient safety during therapy sessions, maintain accurate records, and comply with regulatory and facility requirements.
- Provide education to residents and families about home exercise programs, fall prevention, and strategies to maintain gains after discharge.

Requirements
- Active physical therapy license to practice in Maryland.
- Experience in geriatric rehabilitation or skilled nursing facility settings preferred.
- Strong communication and teamwork skills; ability to collaborate across disciplines.
- Ability to work in a skilled nursing environment and commit to a 13-week travel assignment.
- Knowledge of documentation standards and regulatory requirements for physical therapy.
