Minimum Requirements

Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program, or equivalent professional degree (required).
Current State License as a Physical Therapist in MA or compact/licensure that qualifies to practice in MA (required to start assignment).
Proof of current CPR certification (required).
Current driver’s license, reliable transportation, and proof of auto insurance (required).
1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting (required).
Previous home health experience (preferred).
Experience with neuromuscular, neurological, and orthopedic treatment planning and durable medical equipment knowledge (preferred).
Ability to perform physical demands of field work including lifting, bending, standing and prolonged walking (required).
Ability to read, write, and speak fluent English (required).

Assignment Details

Facility Type: Home Health.
Shift Name: Other.
Shift Pattern: 5x8.
Start Date: Assignment begins April 2026 and extends through August 2026.
Role: Provide in-home physical therapy services, develop and revise Plans of Care, document progress notes, and collaborate with interdisciplinary team.
Clinical Requirements: Must perform patient evaluations, instruct on assistive devices, order rehabilitative equipment as needed, and apply infection control/universal precautions.
Travel Requirement: Must be able to travel locally to patient homes and work a flexible schedule.
