Your next opportunity as a Registered Nurse in home health starts here. We are seeking a clinically confident RN with strong OASIS documentation ability for a full-time, 13-week contract assignment. Work within an established care team while providing in-home nursing that directly supports the wellbeing and independence of patients.

Schedule:

• 13-week contract with possible extension
• Full-time, Monday through Friday
• On-call responsibilities may be required
• Daily visit expectations

Key Responsibilities:

• Deliver high-quality in-home nursing care and interventions
• Conduct thorough patient assessments and document OASIS data accurately and on time
• Work collaboratively with physicians and medical team members to implement individualized care plans
• Complete wound care, manage medications, educate patients, and monitor chronic conditions
• Communicate promptly about changes in patient status to the clinical team

Qualifications:

• Active RN license
• At least 1 year of recent RN experience in home health
• Proficient with OASIS documentation (required)
• Experience using HomeCare HomeBase (strongly preferred)
• Proven ability to manage a set, full-time visit schedule
• Reliable transportation and a valid drivers license
