Job Title: Home Care Coordinator Location: 1877 Webster Avenue, Bronx, NY Schedule: Full-Time Pay: $25–$27/hour We’re hiring a Home Care Coordinator to join our fast-paced, mission-driven team in the Bronx. This role is all about making a difference — helping patients receive the care they need by staffing and coordinating home health aides across multiple cases. What You’ll Do: Schedule and assign home health aides to patient cases Communicate with patients and their families to confirm care arrangements Respond quickly to changes, emergencies, or last-minute staffing needs Use HHAeXchange to manage scheduling and documentation What We’re Looking For: Experience using HHAeXchange Bilingual in English and Spanish (required) Strong communication and problem-solving skills Comfortable working in a fast-paced office environment Join a team that truly cares — and where, every day, your work helps improve lives. #J-18808-Ljbffr
