Qualifications & Requirements Minimum 24 months CVICU experience (recent in last 12 months) – REQUIRED Life support certifications from AHA, ARC, or ASHI – REQUIRED No hits on state license to practice – REQUIRED RTO cannot exceed 7 days; no RTO during the first week of assignment – REQUIRED Candidate driver’s license to verify residency – REQUIRED Travel experience – HIGHLY PREFERRED Profile & Documentation Requirements Complete Banner Cover Page – REQUIRED Resume with all facilities, departments worked, trauma level designation, and number of beds listed – REQUIRED CVICU skills checklist completed in last 12 months – REQUIRED Active license to practice verified in last 30 days – REQUIRED Active ACLS certification from approved vendor – REQUIRED Active BLS certification from approved vendor – REQUIRED Driver's License – REQUIRED Certification & State License ACLS (AHA)
