Key Responsibilities\n- \n
- Conduct thorough assessments to evaluate clients' social, emotional, and environmental needs. \n
- Develop individualized service plans that outline specific goals, intervention strategies, and measures for progress evaluation. \n
- Provide one-on-one and family counseling to help clients navigate challenges related to serious illness and psychosocial stressors. \n
- Collaborate effectively with clients to connect them with appropriate community resources, tailored to their specific situations. \n
- Implement crisis intervention techniques, conduct risk assessments, and facilitate safety planning when necessary. \n
- Engage proactively with community organizations and social service agencies to coordinate services and advocate on behalf of clients. \n
- Lead community outreach initiatives aimed at raising awareness about agency services and enhancing accessibility for underserved populations. \n
- Participate in case conferences, team meetings, and continuing education sessions to foster team cohesion and professional development. \n
You will work primarily with patients receiving hospice care and their families, addressing their emotional and social needs during challenging times. Qualifications\n- \n
- A Master's degree in Social Work from an accredited institution. \n
- Current active state licensure as a Licensed Clinical Social Worker (LCSW). \n

- Demonstrated excellence in communication, cultural competence, and problem-solving abilities. \n
