Company‑paid life insurance. Employee Assistance Program. Qualifications & Attributes Licensed Practical Nurse with an active license in the state of employment. Minimum 1 year of experience as an LPN in an acute care setting (2 years preferred). Home care experience desired. Strong communication skills and ability to create positive impressions with patients, families, physicians, and coworkers. Calm, patient, and accommodating demeanor. Compassionate and caring when working with patients. Knowledgeable of nursing best practices and able to make appropriate nursing judgments. Ability to identify situations and handle them with the best solution. Detail‑oriented and observant. Disciplined work ethic with ability to prioritize and be timely. Ability to follow directions and work as a team member. Valid driver’s license and auto insurance with your name as a listed driver. Maintains active LPN license in the state of employment. Major Areas of Responsibility Provides skilled nursing care in a patient’s home as ordered by the attending physician. Performs care following order sets and goals made by RN. Observes and monitors patient conditions. Notifies RNCM of any changes in patient condition. Administers medication as prescribed by the physician. Counsels patient and family on disease/injury processes and management. Incorporates patient and family in development of plan of care. Helps decrease re‑hospitalizations by teaching patients preventative measures and promoting good decisions. Listens to patient and family members. Completes all clinical documentation per agency protocol and Medicare/Federal guidelines. Communicates with scheduler for any changes outside normal schedule. Participates in on‑call requirements and case conferences. Attends in‑service trainings and mandatory agency meetings. Maintains a professional appearance as a representative of the company. Understands and practices agency policies and procedures. Essential Functions Drives to patient’s primary location per scheduled visit. Daily attendance at assigned visit locations. Documents all aspects of subsequent visits within 24 hours to meet Medicare guidelines. Completes and submits all required documentation within specified company requirements. Fulfills LPN visits within scope of practice for a Licensed Practical Nurse. Fulfills LPN visits in assigned geographic location per patient need within federal and state specifications. Adheres to Medicare guidelines for documentation and timing.
