Applies previous nursing experience and base of knowledge and adapts with the patient and family in a home setting .Assesses for the presence of advanced directives and facilitates further action in obtaining information about or implementing advanced directives if indicated .Participates in committees/ meeting.Performs other duties as assigned.ROLE QUALIFICATIONSEDUCATION & EXPERIENCEREQUIREDRN Nursing Degree . Unencumbered RN License in the state of employment or Multi-State license through the Enhanced Nurse Licensure Compact where recognized1 year of demonstrated experienceValid driver license with proof of insurance in applicable statesClear background screeningWorking knowledge of Nurse Practice ActsMust provide valid and current CPR card/certificate that includes in person instructor led hands - on skills checkClear Hep B Screening in applicable statesClear TB Screening in applicable statesSTATE SPECIFIC REQUIREMENTS:Florida:Illinois :Valid driver license with proof of insuranc e: Not required . Staff are prohibited from driving patients .Hep B Screening: Not a prerequisite for hire. Completed as part of the onboarding process.Pennsylvania:Additional Background Screening: Child Abus e, and FBI check .Must complete TB screening with 2 step PPD , or 1 step PPD with proof of 2 step PPD administration within the last year .Texas :N/AKEY COMPETENCIESThe ability to read and communicate in English both verbally and in writing .WORKING CONDITIONS/EQUIPMENT USEWork is performed in the patient’s home - not substantially exposed to adverse environmental conditions .
