Description We are currently seeking a dedicated Case Manager RN to join Big Sur Health Center, a leading healthcare facility in Big Sur. As a Case Manager RN, you will play a crucial role in coordinating patient care, ensuring optimal outcomes, and promoting patient well-being. You will be involved in a variety of projects aimed at improving healthcare delivery and patient satisfaction, making this role essential to our organization's success in providing high-quality care. Advocate for patients to ensure their needs are met and rights are respected. Educate patients and families on healthcare options, resources, and self-care techniques. Conduct regular assessments to evaluate the effectiveness of care plans and make necessary adjustments. Ensure compliance with regulatory requirements and standards of care. Participate in quality improvement initiatives to enhance patient care processes. Requirements: Associate degree in Nursing (RN) from an accredited institution. Active RN license in the state of practice. Prior experience in case management or care coordination preferred. Strong communication and interpersonal skills. Ability to work effectively in a fast-paced healthcare environment. Knowledge of healthcare laws, regulations, and best practices. Excellent organizational and time management skills.
