With general directions, plans, organizes and delivers independent nursing and nutrition counseling to clients and their families in the community, home, and clinic settings.  Successful candidate may be assigned to several clinics on a rotating schedule.  Candidate will assure that all program standards and procedures as outlined by the Illinois Department of Public Health and East Side Health District are met.  Candidate may assist with investigations of communicable disease outbreaks, assess threat to the public health, provide case management services, nutrition education and implement appropriate action to minimize public injury.  Maintaining confidentiality is an absolute must.

Required Skills, Education, & Experience:

«    Bachelor of Science Degree in Nursing.  Must submit a copy of professional license from Illinois.

«    One year experience providing services to families with young children.

«    Must demonstrate the ability to maintain satisfactory working relationships with other employees, patients, family members, health personnel, and other professionals.

«    Capable of working with individuals and groups to promote good health and disease prevention.

«    Requires thorough knowledge, skills and experience working with and utilizing community resources.

«    Requires extensive knowledge of primary, secondary and tertiary levels of prevention and general principles of public health.

«    Must demonstrate the ability to work as a member of the health team.

«    Must have the ability to develop and implement treatment, conduct effective assessment interviews and develop individual care plans.

«    Must have computer literacy at the user level.

Candidate must own a dependable automobile and possess a valid driver’s license and car  insurance
